Step Two
Book an appointment to have your health care practitioner complete and fax in your medical document.
Your health care practitioner can submit your completed form via fax: 1.855.244.9158.

Registration Confirmation
Once we've received your registration and your medical document, you will receive a confirmation email with your Client ID and shop login instructions within 1-2 business days. If you don't receive your confirmation email within 2 business days of your documents being submitted please check your spam folder. If you do not see the confirmation email there, please don't hesitate to contact our team via care@cannafarms.ca.
